一名22歲的張先生，晨間勃起時常感到劇烈疼痛，醫師問診時發現，他之前常進行激烈自慰導致繫帶裂傷，經藥物治療雖有所改善，但每次發生性行為前提槍上陣，還是會感到劇烈疼痛。醫師表示，建議進行包皮環切除手術，以割除過長的包皮，減少勃起時拉扯造成的疼痛。

書田診所泌尿科主任醫師林儒廷表示，繫帶是男性生殖器官中最幽微之處，一旦受傷了男人就有「難言之隱」。由於其為男性勃起中受力最大的位置，因此當繫帶受傷，不論是撕裂傷或是斷裂，只要有勃起現象時，就會感到疼痛不適。

林儒廷曾收治過一名28歲的周先生，3個月前發生繫帶裂傷，雖然使用藥膏治療並接受縫合，卻一直不敢發生性行為。近日，他以為裂傷部位已痊癒，嘗試發生性行為，但勃起時發生疼痛，檢查發現繫帶再次裂傷，經包皮環切手術後，勃起時沒疼痛，裂傷沒再發生。

林儒廷提到，繫帶受傷的求診者多是30歲以下，常發生於行房或男性自慰過度太忘我時，也有人是因拉鍊扯傷。前者可能是因包皮過長造成反覆感染，使得繫帶較易受傷；後者則是外力導致。

治療方面，先以藥物治療，視情況縫合，倘反覆發生裂傷，且有包皮過長者，建議進行包皮環切手術割除過長的包皮，以減少行房時的拉扯及包皮過長易引起感染的衛生問題。

最後，林儒廷提醒，勃起疼痛最常見的原因是繫帶裂傷或斷裂，但包皮裂傷、攝護腺炎、尿道等等也可能會引起勃起疼痛。繫帶裂傷不會有立即的生命危險，但受傷就成了男人的難言之隱，因此倘有勃起疼痛困擾時，應儘早就醫，找出原因治療。
